Skilled Nurse (RN/LVN/LPN) – Wound Care, Trach, and G-Tube Specialist

Our skilled nursing team comprises of dedicated and compassionate RN or LVN/LPN who provide high-quality nursing care to patients requiring specialized services in wound care, tracheostomy care, and gastrostomy tube (G-tube) management. Our nurses have strong clinical skills, attention to detail, and a commitment to patient-centered care.

Key Responsibilities:

  • Perform comprehensive assessments of patients’ physical and emotional needs.
  • Provide specialized care including:
    • Wound care: Assess, clean, and dress wounds; monitor for signs of infection; collaborate with wound care specialists as needed.
    • Tracheostomy care: Maintain trach site hygiene, suctioning, monitoring for respiratory distress, and emergency response.
    • G-tube care: Administer feedings and medications, clean the site, and troubleshoot any issues.
  • Develop and implement individualized care plans.
  • Document patient care accurately and in a timely manner.
  • Educate patients and family members on care techniques and health maintenance.
  • Collaborate with interdisciplinary healthcare team members to ensure holistic care.
  • Adhere to all infection control and safety protocols.

Qualifications:

  • Active RN or LVN/LPN license (as required by state).
  • Minimum of [1–2] years of experience in skilled nursing or acute care.
  • Proven experience in wound care, trach care, and G-tube management.
  • Current CPR/BLS certification.
  • Strong communication and organizational skills.
  • Compassionate, patient-focused mindset.

Preferred:

  • Wound care certification (e.g., WOCN) is a plus.
  • Experience in home health, hospice, or long-term care settings.
